Knee Pain Overview

Knee pain can come from a sudden injury, years of wear, or simple overuse — and each cause has its own fix. Our orthopedic and sports medicine team diagnoses what’s really going on and treats it, from conservative care to minimally invasive procedures, across Long Island and northern New Jersey. We have offices in Plainview, New Hyde Park, and Ronkonkoma, NY, and Clifton, Montville, and Plainfield, NJ.
The short version
- Knee pain is extremely common and has many causes — injury, arthritis, and overuse are the big three.
- Most knee pain improves with conservative care: rest, ice, activity changes, and physical therapy.
- When it doesn’t, targeted injections or minimally invasive arthroscopy can address the specific problem.
- Some symptoms — a locked knee, inability to bear weight, or fever with a hot, swollen joint — need prompt care.
What causes knee pain
The knee is a complex joint that takes a lot of load, so there are many ways it can hurt. Broadly, knee pain falls into three groups:
- Acute injuries — ligament tears (like an ACL), meniscus tears, and dislocations, often from sports or a fall
- Chronic and degenerative conditions — osteoarthritis and long-term wear that cause stiffness and aching
- Overuse problems — tendonitis and kneecap-related pain such as runner’s knee
Where the pain sits is a clue: front-of-knee pain often points to the kneecap or a tendon, inside or outside pain to a ligament or meniscus, and deep aching stiffness to arthritis. Getting the diagnosis right is what makes treatment work.
Knee conditions we treat
ACL Tear
A common sports injury causing instability and swelling, often needing reconstruction in active people.
Torn Meniscus
Cartilage tears that cause catching, locking, or swelling — sometimes treated with rest, sometimes arthroscopy.
Ligament Injuries
ACL, PCL, MCL, and LCL sprains and tears, ranging from ones that heal with therapy to those needing surgery.
Patellar Tendonitis
“Jumper’s knee” — an overuse injury of the tendon below the kneecap, usually treated conservatively.
Patellofemoral Syndrome
“Runner’s knee” — pain around the kneecap that responds well to therapy and activity changes.
How we treat knee pain — step by step
We start with the least invasive option likely to help, following an approach echoed by Cleveland Clinic and Penn Medicine:
- RICE and conservative care — rest, ice, compression, elevation, plus anti-inflammatory medication for flares and minor injuries.
- Physical therapy and bracing — strengthening the muscles around the knee is one of the most effective long-term fixes.
- Injections — a cortisone injection for inflammation, viscosupplementation (gel) injections to lubricate an arthritic knee, or PRP therapy to support healing.
- Minimally invasive surgery — knee arthroscopy for meniscus and cartilage problems, or ACL reconstruction for a torn ligament.
Our approach: Many knee problems never need surgery. We favor the least invasive path that will actually solve yours, and we’ll be honest about whether a procedure is warranted.
Seek prompt care if: you can’t bear weight on the knee, it looks deformed, it locks or gives way, it swells severely right after an injury, or it’s hot and red with a fever (a possible infection). These need timely evaluation.

Frequently asked questions
What’s causing my knee pain?
It depends on the pattern. Injury-related pain tends to be sudden with swelling; arthritis brings stiffness and aching; overuse causes pain around the kneecap or tendons. An exam and imaging pinpoint the source.
What’s the best remedy for knee pain?
For minor injuries and flares, RICE (rest, ice, compression, elevation) plus an anti-inflammatory helps most. Strengthening the surrounding muscles through physical therapy is the best long-term fix for many people.
What’s the best exercise for knee pain?
Low-impact activity and strengthening the quadriceps, hamstrings, and hips take pressure off the joint. A therapist can tailor a program to your specific problem.
Can knee pain be treated without surgery?
Very often, yes. Most knee pain improves with conservative care and, when needed, injections. Surgery is reserved for specific structural problems that don’t respond.
What’s a red flag for knee pain?
Inability to bear weight, a visibly deformed knee, locking or giving way, severe immediate swelling, or a hot red joint with fever all warrant prompt medical attention.
When should I see a specialist?
When home care hasn’t helped after a couple of weeks, the pain is worsening, or the knee is unstable or locking. Early evaluation often means simpler treatment.
