What is a vertebral compression fracture?
A vertebral compression fracture happens when one of the vertebrae — the stacked bones of the spine — cracks and collapses, usually at the front, so the bone becomes wedge-shaped and shorter. They most often occur in the middle back (the thoracic spine). Because weakened bone can give way during everyday movements, these fractures are common in older adults.

Symptoms of a compression fracture
The most common sign is sudden back pain, often triggered by a minor movement like bending, lifting, or even a cough. Other symptoms build up over time, especially when someone has had more than one fracture:

What causes compression fractures, and who’s at risk?
By far the most common cause is osteoporosis — the thinning of bone that makes it fragile. When bone is weak enough, a fracture can happen with little or no force. Less commonly, compression fractures come from a significant injury like a fall or car accident, or from a tumor weakening the bone. The people most at risk are older adults, especially women after menopause, and anyone who has already had one compression fracture. How is a compression fracture diagnosed?
Diagnosis usually starts with an X-ray, which can show a collapsed, wedge-shaped vertebra. An MRI adds detail — it can tell whether a fracture is new or old and whether a nerve is involved. Because osteoporosis is so often the underlying cause, a bone density (DEXA) scan is frequently done as well, so the bone loss driving the fracture can be treated. Diagnosing the fracture and the bone health together is what prevents the next one.
How are compression fractures treated?
Many compression fractures are first managed without surgery, and a good number heal on their own over time. As Cleveland Clinic notes, “some types of compression fractures can heal on their own,” though “healing can take several months” (Cleveland Clinic).
Conservative care
Non-surgical treatment usually includes a period of rest and activity modification, pain-relieving medication, and sometimes a back brace to support the spine while it heals.
